How To Obtain a PSA Death Certificate [Walk-In at Census Serbilis Center or Online thru PSASerbilis]

A death certificate is a document with information relating to the death of an individual. It can be proof that your spouse or relative is dead for burials, inheritance, insurance claims, proof of being a widow, and many more. Here’s a guide on How to get a PSA Death Certificate.

You can get a death certificate authenticated by the PSA at PSA Serbilis Centers, SM Business Centers, or online. Read our guide on how to get a copy of it.

Requirements for obtaining a PSA Death Certificate

People who can get a PSA Death Certificate

  • Nearest Kin
  • Spouse, parent, direct descendant
  • Court of Proper Public Official
  • Duly Authorized Representative (with Authorization letter)

PSA Death Certificate – Walk-in at Census Serbilis Centers

At the main office, you could get a copy as early as on the day, but usually, it would take 2-3 days.

STEP 1: Fill-up the Death Certificate Application Form; you can get it online (check the link) or get a copy at the Serbilis Centers.

How to get a PSA Death Certificate (Walk-In or Online)

STEP 2: Go to a Census Serbilis Centers with your valid ID and get a number from the guard on an officer.

STEP 3: When your number is called, present the documents to the screener. You will then be asked to pay for the fee.

STEP 4: Go to the cashier and pay the fee of PHP 155 per copy. Keep the receipt and the claim stub.

STEP 5: On the releasing date, proceed to the Census Serbilis Center and get a copy of the PSA Death Certificate.

PSA Death Certificate – Walk-in at SM Business Center

In case you are far away from a Census Serbilis Center, then, you can proceed to an SM Mall and go to the Business center to get a copy of a death certificate. This usually takes 7 weeks.

STEP 1: Fill-up a Death Certificate Application Form; it’s also available at the Business center.

STEP 2: Go to the nearest Business Center and get a payment form. Get a queue to pay for the fee.

STEP 3: At the staff, submit your documents and pay the fee that is about PHP 180; it’s 25 pesos higher than the serbilis center as there is a service fee. Keep the official receipt as well as the claim stub.

STEP 4: On the release date, go to the SM Business center to claim a copy of your PSA Death Certificate.

You can also get a death certificate online and get it delivered anywhere in the Philippines or Abroad

STEP 1: Go to this website.

STEP 3: Enter the details where you want the Death Certificate to be delivered. Double-check if the address is correct.

STEP 4: Select “Death Certificate.”

STEP 5: Enter the information asked, then click “Save.” Double-check before proceeding.

STEP 6: Add additional documents you may want. After that, you can click submit.

STEP 7: You will see your request summary. Choose the payment option you want – via online, bank or bayad center.

It would cost is PHP 330per copy for delivery within the Philippine. For abroad, it would cost USD 20.30 per copy.

If you want to do it online, just click proceed to online payment. If not, then you must print the form and proceed to the bank or bayad center.

STEP 8: After payment, you need to wait for the delivery of the Death Certificate.

  • Metro-Manila – 3-5 working days after payment but due to the pandemic it will be 3 weeks after payment
  • Other cities or provinces – 4-9 working days after payment but due to the pandemic it will be 4 weeks after payment
  • Other countries – 6-8 weeks after payments but due to the pandemic it will depend on the resumption and restriction of countries

1. I have paid, can I cancel my request?

No, you can’t. PSA has a no-cancellation policy.

2. Can I request a refund of my payment?

No, you can’t. PSA has a no refund policy.

3. What is considered an unsuccessful delivery?

  • If the one who ordered it could not be found or does not live in the address noted.
  • If the document is refused
  • If the address is not correct or incomplete

4. Can I request another delivery in case it was unsuccessful?

No, as per PSA, an unsuccessful delivery is considered already done. You need to go through the process again.

5. What are the other documents I can get at PSA?

You can get Birth Certificates, Marriage Certificates, and CENOMAR at PSA.
